Free Athletic Physicals for Grades 8 - 12 July 21st

The Saunders Medical Center Clinic will be offering free athletic physicals to the students (grades 8-12) of the Ashland - Greenwood School District on Monday evening, July 21, 2008 at the Ashland - Greenwood High School. You will need to be at the High School lobby to register for the free physical between 4:30 - 6:15 PM, if you fail to register at this time a physical will not be given. The physical examination will begin shortly after you register. Links to the forms needed for this physical are located at the bottom of this article.

If you are unable to attend this date you may call the Saunders Medical Center Clinic for an appointment. Appointments for physicals will be available through July 14th from 2:00 PM until 5:00 PM in the Saunders Medical Center Clinic.

Please be aware of the following items:

* Bring a copy of the AGHS physical form with the personal information completed.

* If not accompanied by a parent or guardian please bring the completed parent consent form with you.

* The physical and consent forms are available in the High School office and the High School weight room or through the links found at the bottom of this article.

* You can still call the Superintendent's Office to register for the physical at 944-2128 through Thursday, July 17th, if you have not preregistered at the high school.
